Who we see
- Adults aged 18 and over
- Seniors aged 65 and over
- We do not see patients under 18. Please ask us and we will point you to a pediatric practice.
- We do not provide gynecology or obstetric care.
We refer to specialists where it is indicated. Some referrals need prior authorization from your plan, and we will tell you if yours does.
Immigration Medical Services
Dr. Chang is a USCIS-designated civil surgeon, authorized to complete Form I-693 for adjustment of status. Exams are by appointment only, with results explained to you in English or Mandarin.

Some applicants for naturalization cannot meet the English language or civics testing requirements because of a medical condition. As a licensed physician, Dr. Chang can complete Form N-648, the Medical Certification for Disability Exceptions, which asks a doctor to describe the condition and how it prevents the applicant from meeting those requirements.
- Review of your medical history and existing records
- Examination and clinical assessment of the condition
- Form N-648 completed and certified for submission with Form N-400
- Explanation of the findings in English or Mandarin
Please note: the certification is a medical opinion, not a decision. USCIS decides whether to grant the exception, and it may still interview the applicant. Bring any records relating to your condition to the appointment so they can be considered.
Medical-Legal Reports
Dr. Chang prepares written medical reports for legal and administrative matters, drawing on decades of clinical practice. Reports are based on an examination and a review of the relevant records, and are written to be read by people outside medicine.
Requirements vary considerably depending on who has asked for the report and what it must address. Please call the office to discuss your matter, what records you can provide, and the timeline you are working to.
Stem Cell Therapy
Dr. Chang offers stem cell therapy at the San Gabriel office. Whether it is appropriate for you depends on your medical history and what you are hoping to treat, so this is a conversation to have in person rather than a decision to make from a web page.
Stem cell therapy is by appointment only. Please call the office to discuss whether this treatment is suitable for your situation, what it involves, and what it costs.
Family Medicine
Dr. Chang continues to provide family medicine care for adult patients, treating the whole person within the context of their life and family.
