What the exam includes
- A medical history, including a review of any medical records you bring
- A physical examination, including an evaluation of mental status
- A blood test for tuberculosis (IGRA). A chest X-ray is also required if that test is positive, and in certain other cases.
- A blood test for syphilis, required for ages 18 through 44
- A test for gonorrhea, required for ages 18 through 24
- A review of your vaccination records against the vaccines the CDC requires for your age

What to bring
- A valid government-issued photo ID, such as your unexpired passport or driver's license
- Your vaccination records
- Any other medical records you have
- Form I-693 with Part 1 filled in. Do not sign it yet: you must sign it in front of Dr. Chang. Download the current edition from uscis.gov/i-693; USCIS accepts only the edition in use when the doctor signs.

How it works
- Call the office to book. Exams are by appointment only.
- Your exam, as described above.
- Dr. Chang completes Form I-693 and gives it to you in a sealed envelope. You, not the doctor, submit it to USCIS. If you file Form I-485 by mail, send Form I-693 in that sealed envelope, unopened. If you file online, USCIS says to open the envelope, upload the form, and keep the original form and envelope until your case is decided.
- Timing, as of September 21, 2026: USCIS says you must submit Form I-693 with your Form I-485; otherwise USCIS may reject your Form I-485. USCIS no longer requires the doctor's signature to be dated within 60 days before you file. A Form I-693 signed on or after November 1, 2023, is generally valid only for the application it is submitted with.
